Paperwork for All Incoming Cadets

There are 6 forms that you must have upon arrival for Freshman Orientation Week to ensure you will receive $1000 uniform commutation in the Spring. Three of the forms you must provide are a copy of your Birth Certificate, copy of your Immunization Record, and a copy of your signed Social Security Card. The other forms can be found below

DA 3425-Medical Fitness Statement

The DA 3425 is a form that you will take to a physician and they will certify that you are physically capable to begin Army ROTC. Ensure this form is taken to your family physician who knows you well enough to make an accurate assessment of your health. This assessment is similar to a sports physical.

Your Physician should be assessing if you can run, do push-ups, sit-ups, wear boots, and your ability to walk long distances while carrying a 35lbs pack

CC 139R

The CC139R is used to obtain personnel data in order to determine eligibility for enrollment and serve as a source document for cadet's service record throughout participation in the ROTC Program. Please only fill out Parts one and two. Only complete Part 3 if you are in National Guard or Army Reserves. The rest of this form will be completed in person during in-processing.

All Incoming Cadets: Text

SF 1199A

The purpose of the SF 1199A is to enroll Cadets into the Army's direct deposit system so that you can be paid. There are no other forms of receiving payment.

Note

The Army must have additional documentation to validate your bank account information. Your financial institution must fill out Section III, and you must provide a voided check. Also, ensure that this account is solely your account.  It cannot be a joint account with a parent. These measures are taken to ensure that no fraud is occurring

The documents below outline key scholarship information that will give you a better idea of your roles and responsibilities as a cadet if you choose to contract, and will give you some information about fitness and nutrition. Your physical fitness is very important you will be conducting a physical fitness test at the end of the fall semester that you must pass to receive the uniform stipend of $1000
