Paperwork for Incoming 4 Year Scholarship Cadets

There are 13 forms that must be completed upon arrival for Freshman Orientation Week to expedite onboarding into Army ROTC. Attached to the link in this section is a PowerPoint presentation that will explain how to fill out these various forms. You are also responsible for completing the forms under the All Incoming Cadets tab

Incoming 4 Year Scholarship Cadets: About

SGLV 8286

The SGLV 8286 is a form that enrolls you in a program of low-cost group life insurance for service members on Active Duty, Ready Reservists, members of the National Guard, and members of the Reserve Officer Training Corps. SGLI coverage is available in increments of $50,000 to a maximum of $400,000

You may opt-out of Life Insurance. If you choose to opt-out you must still fill section 1 and check the last block of section 2, and follow the instructions provided in that block. Finally, sign on page two

DD 93

The DD 93 is a document that allows the Army to identify your emergency contacts and your beneficiaries

DA 3425-Medical Fitness Statement

The DA 3425 is a form that you will take to a physician and they will certify that you are physically capable to begin Army ROTC. Ensure this form is taken to your family physician who knows you well enough to make an accurate assessment of your health. This assessment is similar to a sports physical.

Your Physician should be assessing if you can run, do push-ups, sit-ups, wear boots, and your ability to walk long distances while carrying a 35lbs pack

CC 139R

The CC139R is used to obtain personnel data in order to determine eligibility for enrollment and serve as a source document for cadet's service record throughout participation in the ROTC Program. Please only fill out Parts one and two. Only complete Part 3 if you are in National Guard or Army Reserves. The rest of this form will be completed during in-processing.

The DD 2005 is a document that allows the Army to review personal information needed to facilitate and document current and future healthcare needs

The purpose of the DD 2058 is required for determining the correct state of legal residence for purposes of withholding state income taxes from military pay

W-4 2020 Version

The purpose of the W-4 is to allow the Army to know how much tax to withhold from your paycheck based on filing status, dependents, and deductions

SF 1199A

The purpose of the SF 1199A is to enroll Cadets into the Army's direct deposit system so that you can be paid. There are no other forms of receiving payment.

Note

The Army must have additional documentation to validate your bank account information. Your financial institution must fill out Section III, and you must provide a voided check. Also, ensure that this account is solely your account.  It cannot be a joint account with a parent. These measures are taken to ensure that no fraud is occurring

The documents below outline key scholarship information that will give you a better idea of your roles and responsibilities as a cadet, as well as give information about physical fitness and nutrition. Your physical fitness is very important because you will be conducting the Army Physical Fitness Test on your 4th day of FOW that you must pass to activate your scholarship benefits
